A TDR is filed online for an e-ticket, or at the station for a counter ticket, in the situations the refund rules list: the train was cancelled or ran more than three hours late and you chose not to travel, the AC did not work, you were not given the berth you were confirmed on, you travelled part of the way, or you did not travel at all after the chart was prepared. The deadline runs from before departure to 72 hours after it, depending on the reason.
The claim goes to the zonal railway, which checks it against the train’s record and pays out, in full or in part, typically within weeks. It is not a cancellation: a TDR does not free the berth and does not guarantee a refund.
